Caseworker Form:
In order to request a bag or items for a child or family in Kankakee County, please include gender, size, age, and office location for drop-off. Please give us 24 hours to gather and deliver.
Please include: Age, Size, Gender, and Shoe Size when requesting a bag.
Don't forget: (car seats, baby essential requests, and other items they may need)
Foster Parent / Care Giver Form:
Please include the childs age, gender, size, shoe size and special request. We will do our best to have bags
and items ready within 24 hours for delivery. PLEASE include drop off instructions.
